Personal Information Collection Statement (PICS)
Please read this statement before you provide any personal information to the University Health Service (“UHS”).
1. Purpose of Collection
Our staff members and healthcare providers may ask you to provide personal data (including, but not limited to,
your name, gender, date of birth, identification document type and number, home address, email address, emergency
contact person’s name and contact details, your health condition, past medical history, and medication history).
The UHS may also obtain from any appropriate third party your medical history or relevant information. Such
information is for purposes relating to your healthcare or treatment related purposes or purposes directly related
to any of them including verification of your identity, appointment booking and registration and/or communication
with you in relation to arrangement or provision of services, handling matters related to payment and/or insurance
claims, investigation and processing of enquiries or complaints, clinical statistics and audit.
The provision of your personal data is voluntary. However, if you fail to provide any required or accurate and complete
personal data, it may affect our ability to provide services to you.
2. Transfer and Disclosure of Personal Data
The personal data supplied by you may be transferred to parties who will handle, and/or who may be contacted by us during the handling of, this activity, for the provision of services to you under the care of the UHS, including our staff, and/or any appropriate third parties as we are required by law or regulation for the purposes stated in this statement.
Unless you have given prior consent, the UHS will not disclose your personal data to any third party for purposes other than those stated above or directly related purposes.

保健處職員及醫護人員可能會請您提供你的個人資料（包括但不限於以下類型的資料：您的姓名、性別、出生日期、身份證明文件種類和號碼、通訊資料、電郵地址、緊急聯絡人及/或親屬的個人資料及聯絡資料、有關您的健康狀況、病歷及藥物史等資料）。
保健處亦可能向任何適當的第三方查詢有關您的醫療記錄或有關資料，以使用於有關您的健康護理或治療目的及任何直接有關的目的，包括但不限於核實您的身份、服務預約及登記及/或就該等服務的安排與您聯絡、有關繳付帳單及/或保險索償的事宜、調查和處理查詢或投訴、醫學統計及審核。
提供個人資料給保健處是自願性的，但是，如您未能提供任何所需的或準確和完整的個人資料，將可能影響我們向您提供服務。
2. 轉移及披露個人資料
您提供的個人資料，可能會因應保健處所需向您提供的照護下，交給相關人員或第三方，協助我們為您提供服務。我們的員工或依法需要的合適第三方，都可能在處理過程中使用或接觸到這些資料，以達成本聲明所列的目的。
除非事先獲得您的同意，保健處並不會將您的個人資料披露給用於上述目的或與之直接相關的用途的任何第三方。
