Confidentiality

• All UHS services provided are confidential.
• All communication between you and your doctor are also strictly confidential.

Medical Records

• A confidential medical record is established and maintained for every patient who receives care at the UHS.
• Information is released ONLY with your written consent, upon a court order, or to a second medical provider
  for the purpose of your health management.
